Designed by parents of children with special health care needs and those who work with them, this notebook will help you manage the emotions of having a child with special health care needs. This notebook will also help you to plan and coordinate your child's care and provide you and your family with information on how to partner with doctors and your health care team in order to be an effective advocate for your child.
Circle of Parents® provides a friendly, supportive environment led by parents and other caregivers, where parents are the experts.
HCP programs are located within local public health agencies throughout Colorado. Nurse-led teams provide care coordination for children and youth with special health care needs from birth to 21.
